A Current Sports Physical
Sports Physicals are valid for one year after the date it was completed. We highly recommend student-athletes get their sports physicals with their primary care physician (PCP). In the event, this cannot be done a sports physical may be completed by a physician who is licensed to practice medicine, osteopathic medicine, or naturopathic medicine in any of the states or territories of the United States. Other than a physician, an advanced practice registered nurse, or physician assistant licensed to practice in any of the states or territories of the United States may complete a sports physical. Sports Physical may be turned in to the front office during school hours or to the athletic trainer after school. During Football Season '22-'23 the main athletic training office is located near the baseball field behind the weight room. Once Football is finished the main athletic training office will move to the Ka'u side of the gymnasium. If you have any questions regarding sports physicals please email our athletic trainer at
Athlete Registration
Registering your student-athlete online is required for all sports they may participate throughout the school year. If you student-athlete is returning to Waverider Athletics, you must use the same login as the previous season. The system will not let you create a new account for a returning student-athlete. *Baseline Impact Test
Student-Athlete who participate in a contact sport need to complete a baseline impact test. This is a 30-45 minute test that is taken every two years with the athletic trainer. The following sports require student-athletes to take an impact test (however, it is not required for students to tryout for sport): Baseball, Basketball, Cheerleading, Football, Judo, Soccer, Softball, Track and Field, Volleyball, Waterpolo and Wrestling.
